This form is to be completed by a healthcare professional (MD/DO/ARNP/PA/DC) and is required for students participating in All Sports, Spirit, or Marching Band. After the completion of a preparticipation physical evaluation, this Medical Eligibility Form must be issued to the student/parent(s)/guardian and returned to the school administration. The healthcare professional must retain a copy of the form in the patient's files for their records.
