Middle School Athletic Handbook

This guide provides an overview of the pre-participation documentation required by MSHSAA (Missouri State High School Activities Association) for all sports and activities.

Pre-Participation Documentation – Due Every Year

This form must be completed by a healthcare professional (MD, DO, ARNP, PA, or DC) and is required for students participating in all sports, spirit, or marching band. After the pre-participation physical evaluation, this Medical Eligibility Form must be issued to the student/parent/guardian and returned to the school. The healthcare professional must also keep a copy for their records.
